Shoulder Girdle
Sprain of the conoideum/ trapezoid ligaments
Symptoms:
Sprain of the ligaments is often due to 'smash' movements (forced elevation movements in lateral rotation) in racket sports, baseball and volley ball.The pain is felt in the mid-clavicular and pectoral area.
Findings
- The pain may be provoked at the end of active and passive movements of the shoulder girdle.
- The pain is also reproduced at the extremes of passive movements of the arm but resisted movements remain negative.
- The accessory movement for anterior instability (apprehension test) is also painful.
- During the interpretation of the latter test it is extremely important to differentiate between
- a typical 'apprehension' feeling with considerable pain down the arm and
- clavicular pain at the end of the forced movement together with a normal endfeel.
The latter points in the direction of a lesion of the conoid / trapezoid ligament.
consists of one or two infiltrations with 10-20 mg triamcinolone at the clavicular insertion of the ligaments.
