The Shoulder

Clinical Picture:
- Pain is felt at the tip of the shoulder or in the C4 dermatome posttraumatic or as an overuse injury.
- A special form of AC- strain is the " distal clavicular osteolysis "
- The pain is felt during or after activity.
Pattern:
- Full range of movement with pain at the end of ranges.
- Sometimes in combination with a painful arc which locates the lesion in the inferior acromio-clavicular ligament.
- Local tenderness at the AC-joint if the superior acromioclavicular ligaments are affected.
- An additional test (passive horizontal adduction ) is usually the most painful movement.
