The shoulder: Passive horizontal adduction

Execution:

The patient's arm is brought horizontally in front of the body
At the end of range the elbow is pushed further towards the contralateral shoulder until the endfeel is reached.
The contralateral scapula is fixed.

Indications

To confirm a sprain of the AC joint.
To confirm a subcoracoid bursitis.
As a localizing sign in a subscapular tendinitis (points in the direction of the distal fibres of the tendon).
(The test is also extremely painful in a lesion of the sternoclavicular joint)

Warning
This test will also be positive in a lot of other shoulder
pathologies - capsular patter, subacromial bursitis, supraspinatus tendinitis.
Therefore the interpretation can lead to much diagnostic confusion if the test is performed routinely.
