Treatment of minor instability banks on three pillars

  1. Active fixation of the scapula.
  2. Training of strenght and propriocepsis of the antagonists of the deltoid.
  3. Proprioceptive exercises for the rotator cuff muscles: small weights; high speed = training of acceleration and deceleration.

    in supine lying; role of the capsular feedback for infraspinatus and subscapularis

    side lying for supraspinatus: abduction with the arm in maximal medial rotation

    standing: acceleration- deceleration exercises, with increasing velocity, for the subscapularis and infraspinatus; the deltoideus is maximally relaxed.

Remarks:
  1. It is important to start the exercises only when the routine basic clinical examinaton is painless.
  2. Use small weights, especially in the beginning; the exercises should be performed as relaxed as possible.
  3. Ask the patient to repeat the exercises as often as possible during the day in order to attain a maximal neuromuscular feedback.
