Pain at the shoulder on resisted extension of the elbow seldom results from a triceps lesion but from a lesion between one of the tuberosities and the coracoacromial roof.
On contraction of the triceps the head of the humerus moves towards the acromion and can pinch lesions laying there.
Therefore pain on resisted extension of the elbow has the same meaning as a painful arc (and therefore their presence is often combined).
Painless weakness on extension of the elbow is mainly the result of a C7 root palsy caused by a C6 posterolateral disc lesion.
The resisted adduction of the arm and the resisted flexion of the wrist may be reduced in power.
