Isolated Anterior Capsular Contracture

Differential diagnosis:

Subcoracoid bursitis:

Pattern: also a painful limitation of the lateral rotation.

Apart from a softer endfeel on lateral rotation in a subcoracoid bursitis the D.D. is made on two accessory tests:

• The lateral rotation in 90° of elevation is also limited in an anterior capsular contraction but free and painless in a subcoracoid bursitis.
• The passive horizontal adduction is completely painless in a capsular contraction but very painful in a subcoracoid bursitis.
