Clinical presentation

There is a full range of passive movement, sometimes in combination with some pain at the end of the passive lateral rotation.

Resisted medial rotation is painful but strong.

The lesion is always on the tenoperiosteal junction on the lesser tuberosity. The folowing options are possible:

cranial: in combination with a painful arc - the result of a temporary impingement of the lesion under the acromioclavicular arc.
distal : in combination with pain on passive horizontal adduction - the result of pinching of the lesion between the lesser tuberosity and the coracoid process.
expanded lesion (cranial and distal) : characterised by both a painful arc and pain on passive horizontal adduction.
