Capsular Pattern

Definition:
The capsular pattern is a proportional limitation of the three passive scapulohumeral movements. There is some limitation of abduction, more limitation of external rotation and less limitation of internal rotation.

external rotation > abduction > internal rotation

Examples:

Pattern	External rotation	Abduction	Internal rotation
Slight capsular	-30°	-10°	end of range
Moderate capsular	-45°	-20 to -30°	-5 to -10°
Severe capsular	-60 to -90°	-45°	-15 to -30°

Meaning
A capsular pattern always indicates a lesion of the joint, whatever its nature may be(= arthritis or capsulitis).
It may be either an acute synovitis or a chronic organised reaction of the fibrous capsule.
