Acute rupture of the infraspinatus tendon.

An acute lesion is characterised by:

• a marked painful arc (impingement of the inflamed tendon remnants between coraco-acromial roof and greater tuberosity)
• and marked pain (sometimes slight limitation) on passive lateral rotation (impingement of the tendon remnants between labrum and greater tuberosity).
• Resisted lateral rotation is weak but painless.
