Type III is not reacting on deep transverse frictions.

A lesion of the tendon of the extensor carpi radialis brevis, level with the radiohumeral joint line and over the head of the radius is a rare condition ( 1% of tennis elbows).

If there is no improvement after six sessions, one should consider the following possibilities:

Are you sure about the localisation?
The palpation of the tendon has to be very precise - to hard a palpation will always provoke some tenderness - and the result should always be compared to palpation of the other side.
Is there a double lesion?
It is possible that, besides a type III lesion, there exists also a type II or IV tennis elbow that must be treated before the pain disappears completely.
Is the technique correct?
Deep transverse friction of a type III lesion is not easy and, when executed technically incorrect, quite painful.
